Health and Detection Program

Welcome to the Health Detection Program. This Program is designed to provide breast cancer screening, follow-up diagnostic services for low income women under 40 who have limited or no health insurance living in the Corona and Norco communities of California. The primary objective of the Health Detection Program, funded through The Foundation for Community and Family Health, is to significantly increase the number of women under 40 who receive breast cancer screening services and diagnostic follow-up, as appropriate. The Foundation for Community and Family Health is funding this multi-faceted Program and the success of this Program depends on the joint partnership efforts with Corona Regional Medical Center, and local authorized providers in Corona and Norco. Program providers have an important role. They join with The Foundation for Community and Family Health to assist underserved low-income women in accessing needed services with the intent of detecting cancer early. Working together, significant steps will be made in teaching women about the benefits of screening and early detection.

The Program strives to combat breast cancer by expanding the number of screenings for breast cancer in Corona and Norco and increasing access to quality cancer detection and treatment services for low income and uninsured low income individuals. Program services are provided at no cost to eligible women under 40. Those who do not qualify but request services will be assessed on an as needed basis.
Our mission is to generate resources and financial assets in order to fund health-related services and enhance the health and well-being of children and families in the Corona and Norco Communities. Our goal is to save lives by preventing and reducing the devastating effects of breast cancer through early detection by funding easily accessible treatment services with special emphasis on low-income and women who do not have health insurance.

Health Insurance Portability and Accountability Act (HIPAA)

Primary Care Providers participating in the Program have the sole responsibility related to providing the patient with information and inform the patients of their rights under HIPAA. The Primary Care Provider will provide to patients a copy of their Notice of Privacy Practices (NPP) describing how their medical information may be used or disclosed and how they may access their protected health information.

The Foundation for Community and Family Health will receive data collected in quantifying totals only for the sole purpose of measuring the number of individuals served by the Health Detection Program. Patient information will not be disclosed or provided.

ELIGIBILITY

Women must meet the following age, residency and financial criteria to be eligible for enrollment in the Program. Criteria is established by The Foundation for Community and Family Health and approved by the Primary Care Provider at the time of first office visit.

Age Criteria

Women who meet the financial guidelines are eligible for the following
reimbursed services:

Women under 40 are eligible for breast cancer screening and diagnostic procedures.

Residency Criteria

Women must have a California address and be residents in Corona/Norco communities to be eligible for the Program. (refer to patient completed application for information)
  If outside Corona/Norco communities, must be approved by The Foundation for Community and Family Health on a as need basis.
